Nomadic Care: care plans patients can follow day to day
Nomadic Care wanted to make care plans practical: clear tasks for patients, and a live view of their progress for the people looking after them. We built a platform with two sides, a web app where patients complete tasks and record health measurements, and a portal where providers create care plans and monitor progress.
The challenge
What wasn’t working
Care plans only work if patients follow them between appointments, and providers can see whether they are.
Nomadic Care needed something simple enough for patients to use every day, and detailed enough for doctors to act on, with patient health data shared with the right doctor through clear, high-level analytics.
It also had to serve organizations, doctors and patients, each with a different job to do on the platform.
The solution
What we built
Patient web app
Patients see their care-plan tasks, tick them off and record health measurements, in an app designed to be easy to use every day.
Provider portal
Health providers create tasks and care plans for their patients and follow each patient’s progress between appointments.
Health analytics for doctors
High-level views of a patient’s health data, so doctors can see trends without wading through raw numbers.
Four roles, clearly separated
System admin, organization, doctor and patient roles, each seeing only what it should.
Built for organizations
Each organization manages its own doctors and their patients on the platform.
Health measurement tracking
Patients record their health measurements in the app, building a history their doctor can review over time.
How it works
How a care plan works on the platform
- 1
Organization sets up
An organization joins the platform and adds its doctors.
- 2
Doctor creates the plan
A doctor builds a care plan of actionable tasks for a patient.
- 3
Patient follows it
The patient completes tasks and records health measurements in the web app.
- 4
Doctor monitors progress
The doctor sees progress and health analytics in the provider portal and adjusts the plan.
The result
What changed
- Patients have one simple place for their care-plan tasks and measurements
- Providers can follow each patient’s progress between appointments
- Doctors get high-level analytics instead of raw data
- System admins, organizations, doctors and patients each have their own role and view
